  • Date of Birth
  • Full Name
  • Policy Number
  • Insurance Company Name
  • Name of State the Policy Originated In

If you're missing some of the information, a good place to look is checking and savings account statements which may show automatic payments to insurance companies.

If you can't find the policy or policy number then your State's Insurance Department may have a website that can help people find lost policies. In New York State, the Department of Financial Services can help with lost policy searches.

For other States, the NAIC website also provides a life insurance company location system that can help you determine which state insurance department to contact for assistance.
